Making sense of the Pachychoroid Spectrum. Episode 10

An important point to highlight in this episode is understanding when the term “pachychoroid” becomes pathological—specifically, when there is compression or attenuation at the level of the choriocapillaris. The pachychoroid disease spectrum goes well beyond central serous chorioretinopathy (CSC); it also includes other key entities such as pachychoroid pigment epitheliopathy (PPE) and peripapillary pachychoroid syndrome. Recognizing these conditions is essential for making an accurate diagnosis. In its acute form, CSC typically resolves spontaneously. Anti-VEGF therapy is not first-line, unless multimodal imaging confirms an associated neovascular component. Systemic treatments, such as spironolactone, are generally not indicated in the acute setting; they may be considered in chronic cases where other options are limited. Alternative treatments like focal laser, subthreshold micropulse laser, and more recently, photobiomodulation, are being explored. However, as of now, photodynamic therapy (PDT) remains the gold standard. There’s a growing need for high-quality clinical trials—especially those with control groups—to better understand the pathophysiology and to validate emerging therapies. The absence of control arms in many studies has led to variability and limited reproducibility in treatment outcomes.
